I .am Audrey Ann Urbanczyk a Licensed Clinical Social Worker (LCSW-R) in New York State. I have worked with individuals to recover from depression, anxiety and other life stressors for 35 years.

Please be aware that I am licensed to work only with clients who are physically present in New York State. I do not participate in any legal cases; therefore, you may need to consider finding a different counselor if you are involved in any court proceedings, including family matters, compensation and disability cases.

I check and respond to messages Monday through Friday during regular business hours as noted on the weekly calendar. Any changes to availability will be noted in advance. I make every effort to respond in a timely manner, usually within 24-48 hours. Thank you in advance for understanding that I may be serving other clients or tending to my own professional and personal responsibilities.

AFTER COMPLETING AND RETURNING THE ATTACHED CONTACT INFORMATION INTAKEFORM, please feel free to access my calendar and schedule a 30 or 45 minute private session that is most convenient for you. THIS IS REQUIRED PRIOR TO SCHEDULING OUR FIRST SESSION. IT IS RECOMMENDED THAT THE FORM BE OPENED ON A COMPUTER AS OPPOSED TO A PHONE. I offer Telehealth sessions. My current hours are as follows: Monday-Friday 10AM – 5:00PM Eastern Standard Time with lunch from 12-12:45PM.

As of January 1, 2022 Congress requires a Good Faith Estimate to be provided to you as part of the No Surprise Act. It is intended to reduce unexpected medical bills by providing an estimated total cost for services. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. BetterHelp is the billing entity for my services, you will have to reach out to them directly, as I do not have access to billing and subscription information. You have the right to receive a written or verbal Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescriptions, equipment, and hospital fees.

If Good Faith Estimates apply to you, make sure your health care provider gives you a Good Faith Estimate in writing at least one business day before your medical service. You can ask your health care provider for a Good Faith Estimate before you schedule a service. Please note you must ask BetterHelp for this. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
